Information Relative to Insurance
IF YOU HAVE HEALTH INSURANCE...
Which covers chiropractic care we require that you pay your co-pay or your deductible
and the portion your insurance will not cover on your first visit. This office will bill you insurance company for the
services rendered. Payment of co-pays and portions not covered by your insurance company are due weekly. If payment arrangements
need to be made, please speak to the office staff or Dr. Lorge. We accept cash, checks, Visa MasterCard and Discover card.
IF YOU WERE INJURED ON THE JOB...
We will bill the State of Washington or Self-Insured Company directly. Please tell the office staff if another
doctor for this injury had treated you.
IF YOU WERE INJURED IN A AUTOMOBILE COLLISION...
We will bill your Personal Injury Protection coverage on your automobile policy, the responsible insurance company
and/or your attorney.
IF YOU ARE COVERED BY MEDICARE...
We request that you "pay as you go," we are not assigned with Medicare. We will bill Medicare and you will be reimbursed directly
from Medicare for any covered charges. If you have a supplemental insurance, we will bill that company after Medicare had paid on the claim.
IF YOU DO NOT HAVE INSURANCE...
Which covers chiropractic, we request that you "pay as you go" or make special financial arrangements with our office staff or Dr. Lorge.
